If you have misplaced your American Red Cross certificate and wish to receive a replacement, please fill out the form below. The more information you can provide, the faster we will be able to locate your student file and issue a duplicate certificate. Please note that there is a $10 per certificate administrative fee. In the case of a California Childcare Preventative Health and Safety certificate with EMSA sticker, the charge is $16. We will contact you within five business days of receiving your form.
