Filling out course records
Please fill out the forms accurately and clearly so that they may be processed quickly. It is very important that the course name and components directly state everything taught in the class (i.e. Standard First Aid with AED/Adult and CPR/Adult and Child).
How to submit course records

Payment
Each time a course record is submitted, it must be accompanied by payment. The administrative fee is $8 per student per course. We accept payment by check, VISA, Mastercard, American Express, or purchase order.
Certificates
Please allow us 10 business days to process completed course records and issue certificates. Certificates will be mailed to the instructor to sign and then give out to their students.
If a student loses their certificate, please refer them to the lost certificate section on our website.
